WAIVER INFORMATION Please read the following information and indicate your agreement in
the form below.
As a Volunteer for Liam's Room, I agree to abide by all of the safety
regulations and rules. I assume all risk and hazards incidental to such
participation, including transportation to and from the activities; I do hereby
waive, release, absolve, indemnify and agree to hold harmless Liam's Room, its
directors, the sponsors and/or organizers of the event for any claim arising
out of an injury to myself, whether the result of negligence or from any other
cause.
